Project Name Nurse-Family Partnership Program
   
Project Description Nurse-Family Partnership is an evidence based program.  This is a unique home visiting program for first-time expectant mothers.  It is designed to help build a strong, happy and healthy family.  A first time mother is paired with a specially trained nurse who will visit her in  her home throughout pregnancy and for the first two years of her child’s life.
   
Eligibility Criteria Women expecting their first baby, less than 28 weeks pregnant, 19 years or younger, low income.
